2008 Forms

All Players must download the Parent/Player Consent Form and Parents Code of Conduct form, fill out and bringwith you at time of sign up.

2009 Parent/Player Consent Form
This form needs to be completed and turned into the leage office for each player on the team.

Parents Code of Conduct
This form needs to be completed and turned into the League Office for each set of parents for every player.

All Cheerleaders must down load and complete this form to turn in at sign up.

Cheerleading Consent Form
This form needs to be completed and turned into he League office for every Cheerleader involved in the League.

All Coaches must download and complete this form to turn into office.

2009 Coaches Form
This is the Coaches form that needs to be completed and turned in to the League Office for each coach.
